In order to provide the correct advice we ask that you read through the following information, follow the instructions and complete the questionnaire fully before returning it to reception. When this is completed it will be checked by reception and passed onto our Practice Nurse. The Practice Nurse will also look through this form and you will be contacted to book an appointment. When dropping in this completed questionnaire please ensure to notify reception of any change of contact details.
Bridge Street Surgery - Travel Health Questionnaire
We ask that you allow as much time as possible between the booking of your vaccinations and travel date. Some vaccinations require a course of injections. We ask that this questionnaire is completed and handed in at least 6 weeks before you plan to travel. If you are travelling within 6 weeks we may not be able to offer this service due to time restraints.
Some vaccinations are not free on the NHS and because we are not a registered travel clinic we do not provide these vaccines:
Yellow Fever
Rabies
Japanese Encephalitis
TB
Tick Borne Encephalitis
The above vaccines would need to be provided by a travel clinic. The nearest is Boots Pharmacy, Doncaster Road, Scunthorpe. www.boots.com/travel-vaccination-health-advice-service.
If it is recommended that you have Malaria tablets you should source them from the travel clinic or a pharmacy as they are not available on an NHS prescription.
To complete this form correctly you will need to visit the website, https://travelhealthpro.org.uk/, this will advise you on the recommended vaccines and will help the receptionist know if it would be quicker and more appropriate for you to attend a travel clinic. This could be due to the vaccinations recommended or the course of vaccinations recommended and our appointments. Your appointment will be approximately 20 minutes long as the Practice Nurse will thoroughly go through the vaccinations with you alongside other travel risks. Your travel appointment will include more than just vaccine preventable risks.
