If you are moving into our area and wish to register click on the link below to open the form. When you have printed the form and completed all of the details please bring the form to the surgery or email it to us at nl.b81063@nhs.net.

We also ask all patients over the age of 18 also complete a new patient health questionnaire. Please make sure both forms are fully completed before handing them into the surgery.

We are experiencing a high volume of new patient registrations. Please allow 4 weeks for your registration to be accepted and processed. You will then be able to book an appointment with the Doctor. Please ask Reception for an online registration form to book appointments and order prescriptions.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be with us and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
