You can register via online form(s) or by filling in a physical copy of the form. If you wish to register by downloading the form please use the relevant link below. When you have printed the form and completed all of the details please bring the form to the surgery or email it to us at nl.b81063@nhs.net.
Non-urgent advice: We also ask all patients over the age of 18 also complete a New Patient Health Questionnaire.
Please fully complete your form before handing it into the surgery.
We are experiencing a high volume of new patient registrations. Please allow 4 weeks for your registration to be accepted and processed. You will then be able to book an appointment with the Doctor. Please ask Reception for an online registration form to book appointments and order prescriptions.
